How Do I Qualify?
Your primary care physician and a Hospice physician certify that you have a terminal illness with a limiting life expectancy. For the purposes of qualifying criteria, a life expectancy of 6 months or less is used. After electing Hospice care rather than curative care, Hospice services begin.
Bereavement
Care does not end after the loss of a loved one. Grief and bereavement care are critical aspects of services provided by our Chaplins and Social Workers. If you have more questions about Hospice, grief or bereavement care, please reach out to us.
